Rennie Grove Hospice Care aims to support patients with life-limiting illness to live where they choose for as long as possible by delivering excellent palliative and end-of-life care based around patients and their families both day and night.
Our model of care
We have been providing this care, built around a 24-hour responsive hospice at home ethos, since 1985. Our model of care is recognised nationally as best practice; our Director of Nursing chairs the National Association for Hospice at Home and we work with other hospices nationwide who wish to develop a Hospice at Home service.
We have captured our service model in two flow charts: one showing the specialist palliative care pathway for end-of-life care delivered in the patient’s home; the other showing the specialist palliative care pathway for the last year of life delivered in the patient’s home.
In 2014 - 2015 our model of care enabled 69% of our patients to die at home as opposed to just 22% nationally.
